Registration Information

Returning Player Registration

On line registration for the 2011-2012 season for returning players only will begin on May 1. This capability will be available between May 1 and June 15, 2011. If you did not register on line last season, they you will be required to set up an account before you can begin the registration process.

In order for returning players to qualify for the reduced fee, they must be registered on-line between May 1 and June 15 and must attend the AGM held on April 27, 2011.

Once your on line registration has been completed, mail your payment and required supporting documents to:

New Player Registration

Registration for new players will take place on Thursday June 16, 2011 from 5 - 9pm at Surrey Sport and Leisure Center, Lower Meeting Room.

The following requirements must be met to complete registration:

1. The player must have reached his/her 4^th birthday by December 31, 2011 to enrol. The season runs from early September to late March.
2. A child may only be registered in person by the custodial parent with whom he/she resides. This residence must be within the boundaries of Surrey Minor Hockey Association. The boundaries are:
   • North: Fraser River
   • South: BC Harbours Board Railway
   • West: Surrey City Limits
   • East: 152 Street (56 Avenue to 88 Avenue); 88 Avenue (152 Street to 160 Street); 160 Street (88 Avenue to Fraser River)

New players who have not played minor hockey

New players who have not registered registered with any other minor hockey association must submit the following:

• player registration form
• copy of player's birth certificate
• 2 current proofs of address (see below for what is acceptable proof of address)
• payment in full of applicable registration fees

Registration is not complete until all requirements are met.

New players who have played minor hockey

New players who have played minor hockey in another association must submit the following:

• player registration form
• player movement form (to be filled out by previous association BEFORE coming to Surrey Minor Hockey Association)
• copy of player's birth certificate
• 2 current proofs of address (see below for what is acceptable proof of address)
• payment in full (applicable registration fee plus $15 player movement fee)

Registration is not complete until all requirements are met.

Payment Options

Cheque or money order payable to Surrey Minor Hockey Association. Please put child's name on bottom of cheque. Sorry, we are not able to accept cash or credit card payment.

A dishonoured payment will be subject to an immediate $35.00 service charge per incident. This includes stop payments. Failure to resolve this situation immediately is subject to the player's registration being suspended, and the potential of not being selected to a team.

Proofs of Address

Any 2 of the following are deemed as acceptable proofs of address.

• hydro, cable, gas, city utility bill (must be entire bill including payment stub)
• credit card statement
• bank statement
• purchase agreement

Any type of telephone or cell phone bills are NOT accepted as proofs of address. All proofs of address must be no older than 2 months.

Registration Rates

Reduced rate is for players who are new to the association (this applies to players who have not played minor hockey AND to those who are transferring in from another association). This rate also applies to returning players who register online between May 1 and June 15 AND attended our Annual General Meeting (AGM).

Regular rate is for returning players who did not attend the AGM or are returning players who register after June 15.

Rep try out fees must be submitted by July 15. There is no acceptance for rep try out after that date.

Registration fees for the 2011-2012 season are as follows (includes one practice and one game per week during the regular season):

*Hockey 1-2-3-4 is an Initiation Program. Players are evaluated and can be placed in a level higher or lower than what his/her age dictates, depending on the players ability. Hockey 1 does not have competitive games, therefore the cost is lower. Should a child whose birth year indicates they should play in Hockey 1, but the ability of the player moves him/her up to Hockey 2, the parent would then be required to pay the difference.
